A personal reflection worksheet for integrating your therapeutic experience and preparing for the next chapterEnding therapy is not simply an administrative stop or a clinical evaluation; it is a significant emotional transition. It involves looking back at where you began, acknowledging what has shifted inside you, mourning what is past, and consolidating what you carry forward.
This worksheet is a reflective companion designed to give you space for meaning-making. It can be used as you approach your final sessions or as you gently consider whether you are ready to conclude your therapeutic work.
Take your time with each section. Write freely, without worrying about formal answers—this is for your own personal reflection and integration.

A Note for Your Final Sessions
